The Following Forms are Necessary in order to secure employment with Three Rivers Youth:
PA Child Abuse Clearance form and online processing $10.00 fee
DPW Form CY-113
FBI Fingerprinting online forms and processing $33.00 fee
Select Department of Public Welfare (DPW) background checks
Print form and take to designated UPS store for fingerprinting

**Paper fingerprint card submission is an additional $7.00 for a total payment of $40.00**
Criminal Background check $10.00 fee
Results may be printed if submitted electronically
